Accord provides case management and support services for individuals who qualify under the Developmental Disabilities (DD) Waiver, Community Alternatives for Disabled Individuals (CADI) Waiver, and the Brain Injury (BI) Waiver. Our case managers work with people to complete assessments, implement an individual service plan, arrange and monitor waiver-eligible services, and consult and advocate on behalf of the individual. We also assist with accessing health care providers, educational resources, and social services.
